  Promising Practices: Foster Youth Transition

Thank you for submitting your Promising Foster Youth Transition Practice to us - someone from our staff will contact you shortly.

In the following form, please describe a program or activity that demonstrates how your Youth Transition Action Team, organization, or cross-agency partnership is working to build a comprehensive youth transition system for current and former foster youth. Sample topics may include youth-friendly One-Stops, WIA eligibility, data collection and sharing, evaluation models, high school academic success, strategies for gainful employment and work-based experiences, support for transitions to college, support for permanent relationships, access to affordable transition housing, and youth involvement in planning and decision-making.  

In one or two paragraphs under each section, describe your promising or effective practice. If you have written materials covering this information you would also like to include, please email them to Lisa Elliott at lelliott@newwaystowork.org.

Promising Practices Narrative


The Challenge or Issue your community was facing. What was the situation that led your team, community, or organization to recognize you needed to try a new approach? Please include specific information about the numbers of youth being served or any other baseline information you may have.




The Process your community used to develop a plan of action. How did you go about convening leaders? Who are the key partners? How did you decide on an approach? Develop a plan? Leverage funding? What conditions were in place that made planning for and implementing the approach you selected possible?





The Strategy and Practice your team developed and implemented. What was the strategy your team, community, or organization developed and implemented? How did you put the program into effect? If you had to reorganize or restructure, how did you go about it? What procedures did you put in place? How did you go about overcoming any obstacles? Marketing your project or program?




Evaluation of your project or program. How do you plan to evaluate the effectiveness of your program or initiative? Specifically, what evaluation questions will you ask? What shared data sources will you use? What data will you gather? How will you gather it?




Evidence of effectiveness.
What data and feedback have you gathered that indicates the program is effective?





Next steps your team is planning.
What Plans do you have for continuing, extending, or expanding the program or practice?




Lessons your team has learned. What lessons have you learned that you think would be valuable to others trying to design and implement such a program?





Links to relevant web pages, tools, forms, etc. Do you have links to a program website? Have you developed any tools that you could share with others? Are there any documents like a progress report that you’d like to include? Note: If you have tools or forms you would like to share that are not currently on a web site, please forward them to us and we will post them.
